Pilot privacy notice
This notice covers only the form for becoming a CautaCare pilot facility. It explains what we collect, why, and what you can ask us to do with it.
Who we are
CautaCare is being developed by Cauta Solutions Group, Accra, Ghana. We are responsible for the details you send through the form. You can reach us at info@cautacare.com.
What we collect
- About you: your name, role, email address and phone number.
- About your facility: its name, type, ownership, region and town, its size, whether it is NHIA-accredited, how it keeps records today, its internet connection, and what you would like to improve.
- How and when you would like us to contact you, when you could start, how you heard about us, and anything else you choose to tell us.
The form does not ask for patient information, and you should not include any.
Why we use it
Only to consider your facility for the pilot, to contact you about it, and to plan the pilot. We use your details because you agree to it when you send the form. We do not use them for advertising, and we never sell them.
Who can see it
The CautaCare team at Cauta Solutions Group. The companies that host our systems and deliver our email handle the details on our behalf and may not use them for anything else. We do not share them with anyone else unless the law requires it.
How long we keep it
Up to 24 months after you send the form, and then it is deleted. If your facility joins the pilot, the details needed to run it are kept under the pilot agreement instead.
Your rights
Under Ghana's Data Protection Act, 2012 (Act 843) you can ask to see the details we hold about you, correct them, or have them deleted, and you can withdraw your agreement at any time. Email info@cautacare.com. If you are not satisfied with our answer, you can complain to the Data Protection Commission of Ghana.
Changes
If this notice changes, the new version will be published on this page. Last updated 29/09/2026.
